a.This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 4
sets of plans,accurate plot plan to scale.Fee according to schedule.
b.Plot plan showing location of lot and of buildings on premises,relationship to adjoining premises or public streets or
areas,and waterways.
c.The work covered by this application may not be commenced before issuance of Budding'Permit.
d.Upon approval of this application,the Building Inspector will issue a Building Permit to the applicant.Such a permit
shall be kept oil the premises available for inspection throughout the work.
e.No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f.Every building permit shall expire ifthe work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months froin such date.If no zoning amendments or outer regulations affecting the
property have been enacted in the interim,the Building Inspector may authorize,in writing,the extension ofthe permit for an
addition six months.Thereafter,a new permit shall be required.

Based on the application your agent submitted and additional New York State Department of
Environmental Conservation (DEC) information, DEC has made the following determination.
The portion of the referenced property located landward of the functioning bulkhead more than 100 feet
in length and originally constructed prior to August 20, 1977, as evidenced on Tidal Wetlands Map#714-
540, is beyond the jurisdiction of Article 25 Tidal Wetlands.
Therefore, in accordance with the current Tidal Wetlands Land Use Regulations (6 NYCRR Part 661), no
permit is required for work occurring at the property landward of the jurisdictional boundary indicated
above. Please be advised, however,that no construction, sedimentation, or disturbance of any kind may
take place seaward of the jurisdictional boundary without a permit. It is your responsibility to ensure that
all necessary precautions are taken to prevent any sedimentation or other alteration or disturbance to the
ground surface or vegetation within the DEC's jurisdiction which may result from your project. Such
precautions may include maintaining an adequate work area (i.e. a 15' to 20' wide construction area) or
erecting a temporary fence, barrier, or hay bale berm.
Please be further advised that this letter does not relieve you of the respo jbility of obtaining any
necessary permits or approvals from other agencies.
