The Permittee James B. Ford

part of the consideration for the issuance of the Permit does understand and prescribe to the fol-
lowing: 1 . that the Southold Trustees Permit must be prominently '
di"splayed'on 'the premises effected thereby during the .period

That the said Board of Trustees and the Town of Southold are release
all damages, or claims for damages, of suits arising directly or indirectly as a result of any oiler- . uncle
t;tion performed pursuant to this permit, and the said Permittee will, at his or her own expense, the
''defend any and all such suits initiated by third parties, and the said Permittee assumes full liability permi
with respect thereto, to the complete exclusion of the Board of Trustees of the Town of Southold -
I.' .That this Permit is valid for a period of 19 mos. which is considered to be the
sufficient time required to complete the work involved, but should circumstances warrant, request
for an extension may be made to the Board at a later date.
4.; That this Permit.should be retained indefinitely, or as long as the said Permittee wishes,
toffiaintain the structure or project involved, to provide evidence to anyone concerned that au&
orization was originally obtained.
5: That the work involved will be subject to the inspection and approval of the Board or
its agents, and non-compliance with the provisions of the originating application, may be cause for
revocation of this Permit by resolution of the said Board.
6. That.there will-be no .unreasonable interference with navigation as a result of the work
herein authorized.
T. That there shall be no interference with the right of the public to pass and repass along
the"beach between high and low water marks.
8. That if future'operations of the Town of Southold require the removal and/or alterations
in,the location of the work herein authorized, or if, in the opinion of the Board of Trustees, the
work shall cause unreasonable obstruction to free navigation, the said Permittee will be required,
upon due notice, to remove or alter this work or project herein stated without expenses to the Town
of Southold.
9 That the said Board, will be notified by the Permittee of the completion of the work auth-
orized. . (See tear off sheet. )
10 . That the Permittee will obtain all other permits and consents that may be required sup-
plemental to this permit which may be subject to revoke upon failure to obtain same.

Dear Judy:
In the matter of the Wetland Application submitted by
Costello Marine Contracting Corp. on behalf of James B. Ford
to construct approximately 75' of bulkhead in front of existing
bulkhead which has deteriorated in Town Creek,.. Trustee Bredemeyer
stated that the Trustees surveyed the referenced_ site and it was
the general accord' on this that the decision on _this should be
consistent with the one that .was granted on- the Goldsmith application
and. that was-.approved in front of the- existing bulkhead, with-- the
provision that in the future, .replacement is .done . in place.
On motion made by Trustee Bredemeyer seconded by.-.Trustee Cochran
it was
RESOLVED to recommend to the Town Board Approval of the Wetland
Application submitted by Costello. Marine Contracting Corp. for
James Ford to construct approximately 75 ' of bulkhead directly
In-front of existing bulkheadwhich. has deteriorated -in Town: Creek,

Pursuant to Article 8 of the Environmental Conservation Law
State Environmental Quality Review and 6NYCRR Part 617, Section
617.10 and Chapter 44 of the Code of the Town of Southold, notice
is hereby given that the Southold Town Board, as lead agency for
the action described below, has determined that the project , which
is unlisted will. not have a significant effect on the environment .
DESCRIPTION OF ACTION
Application of Costello -Marine Contracting Corp.., on behalf of James B. Ford,
for a Wetland Permit to, construct b'ulkhead directly- in front of existing
deteriorated bulkhead at Hobart Road, on Town Creek,' Southold-,. New York..
The project has been determined not to have a significant effect
on the environment for the following reasons:
An environmental assessment has been submitted which indicated
that no significant adverse effect to the environment are likely to
occur should the project be implemented as planned.
Because there has been a response. from the. Board of Southold
Town Trustees and Southold Town Conservation Advisory .Council indicating
that this project would not have a significant effect to the environment.
Because there has been no response in the allotted time from the
New York State Department of Environmental Conservation and the Southold
Town Building Department , it is assumed that there are no objections nor
comments from those agencies.

INSTRUCTIONS FOR PERMIT
A. This application is to be completely filled in by typewriter or ink and submitted to the Town
Clerk in quadruplicate, accompanied by written permission from the owner of the property if not
the some as the applicant.
B. Pilot Plan must be drawn to scale on the reverse side of this application showing the location
of property, the names and locations of Owners of adjoining properties, and a detailed description
of proposed project. (Note: Copies of the Pilot Plan may be made by a copy machine and attached
to the Permit Application)
C. No operation shall be initiated by the applicant until all permits that are required are Issued.
D. A detailed statement of the rehabilitation and proposed condition of the premises after the
work is completed must be given, including a survey if required. }
E. If permit is being sought for a more sizeable excavation than the construction of a private
single dock .or jetty, then this application must be accompanied by a survey and t

certified by a registered land survey or Professional Engineer. The horizontal control survey shall be based upon on approval local coordinate system. The vertical control for elevation and sound-
ings shall be based upon United States Coast and Geodetic Survey and/or United States Geological
Survey datum.
F. A statement must accompany the application describing any known prior operations conducted
on the premises in question and whether any prior licenses or
• structures or to dredge or d p permits have been issued b erect

were ever revoked or suspended by a governmental agency.
G. A filing fee of $25-SB shall acco
mpany the application. This filing 'fee includes one inspection,
there is a $5.00 fee for each additional inspection.
H. The Town Board upon request c t� applicant for a permit, may waive in whole or in part,
the provisions of Article 11, Section93 ;subdivisions A and 0 where it finds that E
