1.That it is the intention of the undersigned to request a Permit from the Board of
Trustees to:

' Construct onto existing 1,424 sf single-family dwelling a cantilevered 2nd floor
addition over approximately 496 sf portion of existing first floor.to remain (plus
1' x 19' overhang), with a 4' x 8' balcony, and a 40 sf covered entry porch addition
with steps (in place of existing entry steps); reconnect renovated roof to existing
stormwater drywell; and maintain existing 10-foot wide non-turf buffer adjacent to
bulkhead, all as depicted on the site plan prepared by Mark Schwartz &Associates
dated June 3, 2026.
2. That the property which is the subject of Environmental Review is located
adjacent to your property and is described as follows:

3. That the project which is subject to Environmental Review under Chapters
96,111 and/or 275 of the Town Code is open to public comment on:
