PART 1:APPLICATION RATING AND CHECKLIST
APPLICATION RATING CRITERIA
Applications will be rated on a number of criteria. Please be as detailed as possible in the following questions
and please provide accurate information as possible in order for the town board and the advisory committee
to evaluate your project to the best of their ability.
Applications approved by the Southold Town Board will be required to enter into a funding agreement with
the Town of Southold. Funding agreements shall be subject to such terms and conditions as the Southold
Town Board may deem appropriate to protect the interests of the taxpayers of the Town of Southold, based
upon a specific project.
DOCUMENT CHECKLIST
❑ Completed Application Form
❑ Project Narrative (Review page 2 for instructions)
❑ Signed Attestation
❑ Signed property owner authorization form (if applicable)
❑ Site map that shows the location of the proposed design
❑ Copy of the Last Recorded Deed to the property where the project is proposed and copy of Mortgage
Lien (if applicable)
❑ Copy of the most recent certificate of occupancy or certificate of use for subject property (if applicable)
❑ Estimates from licensed contractors and any supporting documentation for the project budget
proposal. The Community Housing Advisory Board (CHAR) and the Town board review budgetary
information closely. Please be as detailed as possible when providing the project budget proposal.
❑ Letters of support (if applicable)
❑ Architectural design drawings. These can be schematic/preliminary designs that support the project
narrative.
❑ Documentation providing additional sources of funding.
❑ IRS Form W-9 for preparation of a purchase order if awarded funding.

PROJECT NARRATIVE REQUIREMENTS:
Please provide a Project Narrative as a separate attachment, The Narrative should be no more than 3 pages and must
address the following key points:
1. Project Overview:
o Summarize the proposed development, including the location, type (e.g., single family, multi-
family, mixed-use), proposed method of wastewater disposal, and primary objectives.
2. Feasibility and Readiness:
o Describe the current status of the property(e.g., ownership, zoning compliance) and provide an
estimated project timeline.
o Highlight your team's qualifications and relevant experience in developing community housing.
o Provide at least one example of a successful project you have completed and explain how it has
prepared your team to successfully deliver this project.
o Explain why it is reasonable to expect that this project will be completed as proposed, based on
your qualifications, experience, and organizational capacity.
3. Ownership and Management Structure:
o Identify who will own the property once completed(e.g., developer, non-profit, property
management firm).
o Specify who will be responsible for ongoing management, maintenance, and tenant relations,
including details about their qualifications and experience with similar properties.
4. Operational Strategy:
o Detail how the project will be managed long-term, including maintenance, tenant support, and
community engagement to ensure sustainability.
5. Financing:
o Explain how this project will be funded. What percent will be funded by the community housing
fund?what percentage will be funded by other grants? What percent will be funded by you
personally?

Allocation of Community Housing funds will not be used for any purpose other than those specifically
permitted by Southold Town Code, the Southold Town Housing Fund and the Peconic Bay Region Community
Housing Fund.

PART 6:PROPERTY OWNER CERTIFICATION
Public Property:
Property is owned by a municipality and a resolution from the appropriate agency or board specifically allowing thecompletion of
this project in its entirety has been obtained and is attached to this application.

The applicant is the property owner and certifies that the project will be completed as described. Or
The property owner has provided an attached letter or completed the below certification allowing the access
to the premises for the purposes of planning,designing,constructing,and completing the proposed project as described.
