Mobile Site Name 1HFB Southhold Town Human Resource Center Address 750 pacific Street Mattituck NY 11952 Mobile Site Coordinator Shatina Jayne Contact NaMe Phone&Fax Number 631 298 4460 Email Address shatinaj@southo/dtownny.gov Schedule: Date(s) of CSFP food i 1st 2nd 3rd 4th distribution: Mon Tues Wed Thurs Fri Sat Sun Times(s)of CSFP food k distribution: Distribution 1pm to 3pm Purpose: This agreement is to outline each party's responsibilities in the cooperative effort to provide the I benefits of the Commodity Supplemental Food Program(CSFP)to eligible adults 60 years of age and older at a mobile location.This agreement will serve to enhance the Program by reaching eligible participants who may not otherwise have access to a CSFP agency, and to ensure that proper procedures are in place.An MOU must be in place prior to any CSFP activity. Term: This MOU,is valid for one year and must be renewed annually on the anniversary date of the established MOU. Either party may cancel the agreement with thirty (30) days advanced notice, and cancellations must be in writing. CSFP Local Agency Responsibilities: • Conduct an information session on program eligibility, certification and food issuance procedures for mobile site staff. • Gather all required documents from participants/proxies to complete certification requirements. • Designate a specific delivery date and time to be-agreed upon by the mobile site. • Ensure that CSFP commodities are delivered in good condition and stored properly during transport. ®CamScanner J t • Responsible for,any loss resulting from improper storage, care, or handling of t commodities • Distribute one(1)CSFP food package monthly to each eligible CSFP participant, unless the local agency provides bimonthly issuance. • Remain at the mobile location while commodities are issued. • Serve all eligible participants regardless of race, age, color, citizenship, national origin, ancestry, gender,sexual orientation including gender identity or expression, marital status, religion, unfavorable discharge from the military or status as a protected veteran, €; or disability, E • Display the "And Justice for All Poster"during CSFP distribution and certification. • Distribute nutrition education materials to participants/proxies. • Return any unissued commodities to the local agency the same day of distribution. • Maintain a copy of the MOU agreement. Mobile Site Responsibilities: • Identify staff to attend the informational session on eligibility, certification and food ` issuance. • Inform applicants of documentation requirements(proof of age, proof of current income, proof of current address, photo identification)prior to initial certification. • Provide an area that is secure, maintained at proper temperature and free of infestation for the distribution of food packages. • Ensure that CSFP local agency staff,can access the mobile site location to provide CSFP services at the designated time and date. • Notify participants when the food is delivered. • Serve as a proxy for a maximum of five(5) participants, if desired. • Be responsible for any loss resulting from improper storage,care, or handling of OR commodities. f • Serve all eligible participants regardless of race,age,color, citizenship, national origin, ancestry, gender,sexual orientation including gender identity or expression, marital status, religion, unfavorable discharge from the military or status as a protected veteran, or disability. Inform the CSFP local agency of deceased participants or those who have relocated. • Notify the CSFP local agency immediately if a change takes place in the program address, mobile site coordinator, phone number,days of distribution,email'address, etc. • Agree to unannounced monitoring visits by the CSFP local agency,the Department of Health, and'the'United States Department of Agriculture. • Ensure that financial compensation (i.e.tips, donations, contributions, etc.) is not requested or accepted for the delivery of CSFP commodities, Maintain a copy of the MOU agreement. Civil Riahts Requirements: Both parties hereby agree to comply with: 1.Title VI of the Civil Rights Act of 1964(42 U.S.C. 2000d et seq.) ii.Title IX of the Education Amendments of 1972(20 U.S.C. 1681 et seq.); III. Section 504 of the Rehabilitation Act of 1973(29 U.S.C. 794); iv.Age Discrimination Act of 1975(42 U.S.C. 6101 et seq.); v.Title II and Title III of the Americans with Disabilities Act(ADA)of 1990 as amended by the ` ADA Amendment Act of 2008(42 U.S.C. 12131-12189); A. Executive Order 13166, "Improving Access to Services for Persons with Limited English Proficiency."(August 11,2000); ®CamScanner 1 t l ' 1 vii.All provisions required by the implementing regulations of the Department of Agriculture (USDA) (7 CFR Part 15 et seq.); viii. Department of Justice Enforcement Guidelines(28 CFR Parts 35,42 and 50.3); ix. Food and Nutrition Service(FNS)directives and guidelines to the effect that, no person shall, on the grounds of race, color, national origin,sex, age, or disability,be excluded from participation in, be denied the benefits of, or otherwise be subject to discrimination under any program or activity for which the Program applicant receives Federal financial assistance from USDA;and hereby gives assurance that it will immediately take measures necessary to t effectuate this Agreement. x.The USDA non-discrimination statement that in accordance with Federal civil rights law and U.S. Department of Agriculture(USDA) civil rights regulations and policies, the USDA,.its Agencies, offices, and employees, and institutions participating in or administering USDA programs are prohibited from discriminating based on race, color, national origin, religion, sex, gender identity(including gender expression), sexual orientation, disability, age, marital status, , family/parental status, income derived from a public assistance program, political beliefs,or 4 reprisal or retaliation for prior civil rights activity, in any program or activity conducted or funded by USDA(not all bases apply to all programs). € This assurance is given in consideration of and.for the purpose of obtaining any and all Federal financial assistance, grants, and loans of Federal funds, reimbursable expenditures, grant, or donation of Federal property and interest in property,the detail of Federal personnel, the sale and lease of, and the permission to use Federal property or interest in such property or the furnishing'of services without consideration or at a nominal consideration, or at a consideration that is reduced for the purpose of assisting the recipient, or in recognition of the public interest to be served by such sale, lease,or furnishing of services to the recipient, or any improvements made with federal financial assistance extended to the Program applicant by USDA.This includes any Federal agreement, arrangement, or other contract that has as one of its purposes the provision of cash assistance for the purchase of food, and cash assistance for purchase or rental of food service equipment or any other financial assistance extended in reliance on the representations and agreements made in this assurance. By accepting this assurance, each agency agrees to compile data, maintain records, and submit records and reports as required, to permit effective enforcement of nondiscrimination laws and permit authorized USDA personnel during hours of program operation to review and copy such records, books,and accounts, access such facilities and interview such personnel as needed to ascertain compliance with the nondiscrimination laws.
