BE IT RESOLVED that the Southold Town Planning Board authorize the Chairman to endorse the final surveys dated November 6, 1991, and Phase I of the site plan dated March 16, 1992 and signed by Roderick Van Tuyl, L.S. All conditions of final approval have been met with the exception that the Health Department's approval does not include Phase 2 of this site plan. Consequently, no building permit may be issued for any part of Phase 2 until a survey with Health Department approval is received by the Planning Board and this site plan is resigned by the Planning Board Chairman. Town Hall Southold, N.Y. 11971 Ref; Peter & Maryanne Hennessey Dear Mr. Orlowski, Reference the above properties, it is the opinion of this board that a new fire well be placed as marked by an X on the enclosed drawings. The Fire Well is to conform to our specifications and be equipped as follows; 1, 20 H.P., 3 Phase, Submersible pump capable to deliver 400 GPM at 10 PSI discharge at center of tee at top of well. Pump shall be of submersible turbine type, equipped with grease packed bearings in bowl assemblies. The bowls shall be of a smooth finish design to guide the water from one stage to the next with a minimum of turbulence. 2. Top of the well is to be finished off with an 8" x 4" well seal. 4" galvanized tee and plug, and 4" x 4-1/2" solid brass ferrule and cap. In our previous correspondence the Board requested a restrictive covenant be part of the requirements. PO Box 930, Cutchogue, NY 11935 Telephone (616) 734-6907 • Fax (616) 734-7079 l ~ March 22, 1993 ®p Buccaneer Books Mr. & Mrs. Hennessey v Depot Lane Cutchogue, NY 11935 Ref; Fire Well requirements Dear Mr. & Mrs. Hennessey, At a special meeting of the Board of Fire Commissioners on March 22, 1993 the following resolution was adopted and a copy sent to the Southold Town Planning Board. 1) Be it resolved to provide adequate fire protection to the building(s) on the subject property, the Board of Fire Commissioners is continuing to recommend that a fire well be installed as outlined in the letter of the Cutchogue Fire District's secretary, dated December 26, 1991, to the Chairman of the Southold Town Planning Board. 2) Be it resolved that it is further recommended that the construction of this fire well is primary for the benefit of the subject property, however due to the residual benefit to the existing structures and buildings in the surrounding neighborhood, that 75% of the cost of this fire well be paid by the applicant and the balance by the fire district. 3) Be it resolved that it is further recommended that as a condition of any site plan approval and Certificate of Occupancy for the presently constructed building that the land owner agree to an annual inspection by the Chief of the Cutchogue Fire Department to determine the type of materials stored in the warehouse so that the fire district will have adequate notice on how to be prepared to handle an emergency at this location. 4) Be it resolved that it is further recommended that prior to the issuance of site plan approval or Certificate of Occupancy to the present building that a restrictive covenant be recorded in the Suffolk County Clerk's office to reflect the above resolution. The above resolution was made by commissioner Robert Doroski, seconded by Commissioner John Bakowski and carried by a majority of the Board. The Fire Well is to conform to our specifications and be equipped as follows; 1, 20 H.P., 3 Phase, Submersible pump capable to deliver 400 GPM at 10 PSI discharge at center of tee at top of well. Pump shall be of submersible turbine type, equipped with grease packed bearings in bowl assemblies. The bowls shall be of a smooth finish design to guide the water from one stage to the next with a minimum of turbulence. 2. Top of the well is to be finished off with an 8" x 4" well seal. 4" galvanized tee and plug, and 4" x 4-1/2" solid brass ferrule and cap.
